Rates: My practice is a combination of direct-pay and health insurance benefits. I am happy to assist you in either billing your insurance company or providing you with a detailed invoice, so that you'll be reimbursed for part or all of your session.
Insurance: Services are usually covered in full or in part by your health insurance company. Please check your coverage carefully by asking the following questions:
Do I have mental health/behavioral health coverage?What is my deductible and has it been met?How many sessions per year does my insurance cover?I accept the following insurances:
Anthem Blue Cross HMO/PPO
All other PPO plans including Aetna, Pacificare, Blue Shield, Cigna, and United Healthcare
Payment: Cash and checks are accepted for payment.
Cancellation Policy: If you do not show for your appointment or cancel at least 24 hours ahead of time, you will be required to pay for the full cost of the session.
